Patient Care Specialist OpportunityAs a Licensed Practical Nurse with Prisma Health, you will play a vital role in delivering high-quality patient care in our Palliative Medicine department. Your expertise will be instrumental in ensuring the comfort and well-being of our patients.Key Responsibilities:

• Assist physicians with examinations, procedures, and other activities related to direct patient care.
• Arrange for patient appointments for diagnostic testing, consults, referrals, and admission to hospital.
• Monitor and maintain stock of medical supplies and proper functioning of equipment.
• Document patient's vital statistics and other encounter data into patient chart and ensure that all required records are in the chart.
• Obtain brief medical history and chief complaint from patients.
• Call in prescriptions for patients according to physician order.
• Promote positive health behaviors and self-care skills through patient education.
• Prepare exam rooms with appropriate instruments, supplies, and equipment.
• Receive and return clinically related telephone calls; screening and forwarding those requiring physician action.
• Complete medical/physical history forms and other related information for insurance companies.
• Inform physician of changes in schedule.Qualifications:Completion of a LPN program recognized by the SC State Board of Nursing. No experience required.Licensed to practice as a LPN in South Carolina. In lieu of completion of a LPN program, will accept program equivalency recognized by the SC State Board of Nursing.
