Job Summary

Transform the health of our communities, one person at a time, as a care management team leader. This role oversees the care management team and develops high-quality, cost-effective healthcare outcomes.

Responsibilities

• 
  
• Provides guidance on care management documentation for federal and state compliance
  
• Develops and maintains care management policies and procedures
  
• Sets goals and objectives for the care management team
  
• Partners with internal and external stakeholders to address member needs
  
• Manages process improvements for cost-effective healthcare results
  
• Educates care management leaders on key initiatives and member outreach
  
• Collaborates with resources to assess member needs
  
• Trains care management team members for high-quality care
  
• Develops an overall strategy for onboarding new team members
  
• Leads change within scope of responsibility
  
  
  Requirements
  
  
  • 
    
  • Graduate from an Accredited School of Nursing or a Bachelor's degree
    
  • 6+ years of related experience, including prior management experience
    
  • RN - Registered Nurse - State Licensure and/or Compact State Licensure required
    
  • Alternate Licensure applicable - RN, LPC, or MSW, LCSW in South Carolina
